ATTENTION Parents/Guardians of Seniors:
All students entering twelfth grade this fall will need to show proof of receiving two doses of meningococcal conjugate vaccinations (MCV,). This vaccination will prevent your student from a severe form of bacterial meningitis as well as other illnesses caused by this bacterium.
A meningitis vaccination is documented as “MCV4,” “Menveo,” or “Menactra” on an immunization record. Accepted documentation to comply with the requirement for students entering 12th grade includes at least one of the following:
Letter or note signed by your healthcare provider identifying the date of the meningitis vaccination
Printout from your provider’s electronic medical record that indicates the vaccine was administered
Current Certificate of Child Health Examination form, specifying the date the meningitis vaccination was administered and signed by health care professional
Copies of documentation may be dropped off in the Niles West Health Office, faxed to 847-626-3520, mailed to the Niles West Health Office, 5701 W. Oakton Ave, Skokie, IL 60077, or emailed to the school nurses David or Dawn at [email protected] or [email protected] Please be sure to maintain a copy for your personal records. Your student will need a copy of current immunizations and proof of having received the meningococcal vaccine for matriculation into college.
If proof of vaccination is not received by October 15, 2017, per state law, your student will be excluded from school until proof is received.
In addition to your health care provider, many local pharmacies offer the vaccine. The Skokie Health Department offers vaccines to uninsured and underinsured residents, please contact for appointments: 847-933-8252.
